How can I upload an image and reference it in CSS?

The structure of the image reference should be www.yourdomain.com/files/filename.filetype.

For example, if you have uploaded a file named 'sample.jpg' to the file manager (Click Manage then Settings then Files) the direct link to access that file would be: www.domainname.com/files/sample.jpg.
